have a look again at the sample report. in particular look at the output of the shared variable.

note that there are two values being rolled up in shared stringvar osrt. e.g.
1 & $37,026.11 in the first instance,
2 & $56,994.06 in the second.
i can see in the error that you haven't followed this format. where is the description / doctor name? there are 0 description values added to your array and not 11. this is the cause of the error.
to solve this go back to your subeport and the formula that rolls up the values. ensure that you see a description appearing in the shared variable of the main report.
